Job Description

We are seeking a compassionate and detail-oriented Intake LPN to support our Home Health intake and referral process. This role involves coordinating with referral sources, evaluating patient eligibility, and ensuring a smooth intake process.

Key Responsibilities:

Communicate with referral sources to facilitate a timely intake process.
Evaluate patient eligibility and assist with insurance verification.
Provide administrative support to clinical staff and field teams.
Ensure accurate documentation in electronic medical records.
Participate in performance improvement and professional development activities.

Pay: Starting at $27.30/hr.+ based one experience

Weekend: Saturday and Sunday

Qualifications:

Active LPN License (State of Kansas).
Minimum 1 year of nursing experience (home health experience preferred).
Strong interpersonal and communication skills.
Valid driver’s license and reliable transportation.
