About HomeBase HomeBase is a CAMBA program offering case management and essential services to at-risk individuals and families in Brooklyn and Staten Island to prevent homelessness. Services include assistance with public benefits, housing, education, healthcare, and other issues affecting household stability. Position Details Title: Case Manager Reports To: Program Supervisor Location: 2244 Church Avenue, Brooklyn, NY 11226 Responsibilities Prescreen clients over the phone for eligibility and schedule intake appointments. Conduct initial assessments and periodic reassessments of clients and their families. Review documentation for eligibility and maintain client files. Assist clients with completing intake applications and forms. Refer clients to appropriate internal and external services. Follow up with clients and referral organizations on progress. Assess living conditions through home visits/inspections. Prepare marketing materials and input data into databases. Recruit clients by community outreach. Support clients in overcoming barriers and advocating for themselves. Monitor progress, document findings, and determine case closures. Meet program goals and submit required reports. Complete subsidy applications and requests for financial assistance. Minimum Qualifications Bachelor’s degree (e.g., B.A., B.S.W.) with two years relevant experience, or High School Diploma with four years of case management experience in social services. Additional Requirements Proficiency in Microsoft Office (Excel, Word, Outlook). Strong writing and communication skills. Ability to apply basic math concepts. Problem-solving skills and adaptability. Proficiency in a second language (e.g., Haitian/Creole, French, Spanish, Russian). Knowledge of HRA benefits, NYC housing programs, and community resources. Compensation & Benefits Annual salary: $48,410 - $52,530, plus comprehensive benefits including health insurance, dental, 403(b), paid time off, and holidays.
